I understand that the Port Credit Yacht Club (PCYC) is responsible for supervision of my child during class time only. If I cannot supervise my child outside these times, it is my responsibility to arrange for alternate supervision.
Personal Identification Information
PCYC requests personal identification information in connection with your use of, or participation in, registration for PCYC Learn to Sail Programs and in connection with other activities, services or resources. PCYC will never intentionally disclose any personal identification information about you as an individual user to any third party without having received your permission.
PCYC will not sell, rent or lease its member lists to third parties. PCYC may however share data with trusted partners such as Ontario Sailing or Sail Canada to help us perform statistical analysis to better serve the sport. In such a case, we request that the partner limit the use of your personal information to the provision of the services and they are requested to maintain the confidentiality of your information.
The following are exceptions regarding the disclosure of your personal information: we will disclose your personal information, without notice, (a) to comply with any valid legal inquiry, order or process served on PCYC; (b) to protect and defend the rights or property of PCYC; and, (c) to protect the personal safety of users of PCYC, or the public. We may also transfer user information, including personally identifiable information, to a successor corporate entity in connection with a corporate merger, consolidation, sale of assets or corporate division or other fundamental corporate change.
If you would like to review any personal information, we have retained about you, please send us an e-mail request for this information at firstname.lastname@example.org. Once you have reviewed this information, at your request we will correct personal information about you that you state is erroneous.
From time to time, PCYC will take photo or video to depict accurately the activities and programs offered. Any photograph or video may be included in social media, websites, promotional material or advertisements relating to program activities. The use of photographs or video are for PCYC usage only and will not be sold to third parties.
CODE OF CONDUCT
PCYC instructors and coaches follows the Code of Conduct issued by Sail Canada, the National governing body for sailing in Canada. The code can be found here: https://www.sailing.ca/files/2018_Sail_Canada_Coaches_Code_of_Conduct.pdf
While every effort is made for a safe environment, accidents do occur. With regards to Concussions, PCYC will follow the policy developed by Sail Canada, The policy can be found here:
PCYC members will have the membership discount applied when the LTS fees are posted to their account which is in the month that the registration is completed. All accounts shall be due when rendered. If unpaid within 30 days from the statement date the account will be subject to interest from the statement date at the rate set by the Board of Directors. In addition, member privileges may be suspended, and other actions taken as outlined in Club Policy Administration A1.
RELEASE AND INDEMNITY AGREEMENT
I have read the rules and regulations regarding cancellations, wait lists and participant responsibility as laid out online at http://www.pcyc.net and its subpages and agree to be bound by them. In consideration of my child being permitted to take part in the program, I agree to save harmless and keep indemnified Port Credit Yacht Club, and their respective agents, officials, servants and representatives from and against all claims, actions, costs, expenses and demands in respect to death, injury, loss, damage to person or property, howsoever caused, arising out of or in connection with taking part in the PCYC Learn to Sail Program, notwithstanding that the same may have contributed to or caused or occasioned by the negligence of the same bodies or any of them, or their agents, officials, servants or representatives. I further understand and agree that this release is binding upon myself, my heirs, executors and assigns.
