a) Youth Video Waiver / Media Consent For if my course attendee is under 18 years: I hereby authorize any images or video footage taken of my youth (under 18 years of age), in whole or in part, individually or in conjunction with other images and video footage, to be displayed on the RStLYC Website and other official channels, and to be used for media purposes, including promotional presentations and marketing campaigns. I also authorize the display and use of any media material created by my youth within the RStLYC. I waive rights to privacy and compensation, which I may have in connection with such use of my youth’s name and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith. I am over 19 years-of-age and the parent or legal guardian of the youth, and I have read this waiver and am familiar with its content. a) Formulaire de consentement pour les mineurs: Si la personne inscrite est mineure: J’autorise, par la présente, la diffusion de toute image ou vidéo de mon enfant mineur, en tout ou en partie, individuellement ou avec d’autres images ou vidéos sur le site Web de YCRStL et sur d’autres sites officiels, ainsi qu’à des fins médiatiques, y compris lors de présentations promotionnelles et de campagnes de publicité. J’autorise également la diffusion et l’utilisation de tout matériel média créé par mon enfant au nom de YCRStL. I also authorize any media material created by me within the RStLYC. I waive rights to privacy and compensation, which I may have in connection with such use of my name and likeness, including rights to be written copy that may be created in connection with video production, editing and promotion therewith.
